Private Practice Record

Use plain text or paper. A tracking app is optional. Do not publish personal schedules, identifying details about peers or private circumstances.

  • Date, current learning question and artifact location:
  • Starting attempt or plan, before consulting the example:
  • Prediction and the rule used to judge the result:
  • Action taken and actual observation:
  • Source or person consulted; assistance received:
  • Revision and why it follows from the evidence:
  • Unresolved limitation and next check:
  • Focused minutes (read / practice / assess / repair); waiting time separately:
  • Status: planned, attempted, repaired or verified; reviewer or solo simulation:

Required module evidence: An audience-specific explanation, a focused help request, a feedback response and a traceable revision; label real peer work or solo simulation honestly.

If a field does not apply, say why. Never fill a future observation with a predicted result. A reviewer may see a redacted record; the educational decision must remain understandable.
